A form that is filled in by the PRODUCER before any form of filming takes place to assess the potential of the location to cause injury to anyone on the production. Risks are present in all locations and the studio e.g. filming in a boat on a lake requires special safety measures to make sure the risk of drowning or injury is minimised.
